Children's names, ages & medical needs
Name and age of each child, plus allergies, medications, and any behavioral notes the babysitter needs before the parent leaves.
Address & hourly schedule
Where care takes place, and how each period of care is agreed — a start and expected end time, arranged occasion by occasion rather than a fixed weekly shift.
Routines & house rules
Meals, bedtime, screen-time limits, homework expectations, visitors, and driving — the day-to-day instructions that prevent “I didn’t know that was the rule” disputes.
Emergency contacts
A backup contact if the parent can’t be reached, plus the child’s pediatrician and preferred hospital or urgent care.
Authorization to seek emergency treatment
Written permission for the babysitter to consent to emergency medical care if neither parent nor backup contact can be reached in time — without it, a sitter can be stuck unable to authorize urgent treatment.
Hourly rate & payment
The rate per hour, when it’s paid, and any different rate for late nights, holidays, or extra children.
Cancellation / no-show policy
How much notice a free cancellation needs, and what’s owed if the parent cancels late or the sitter doesn’t show.
